Cloth lined 14" x 10" size cover affixed with Rs.40/- postal stamp. (Self-addressed as in Residential proof) |

Fill the application and tag all the documents in sequence as per CHECKLIST |

Submit application IN PERSON in the office of Andhra Pradesh Pharmacy Council. |

Get Originals endorsed & stamped to complete application process. |
