Application - 3 papers  

 

+ Copies of following documents

School final Certificate (SSC etc)

3.

Intermediate Certificate

4.

D. Pharmacy Certificate issued by Board, I, II year marks lists and Course study or Bonafide Certificate or Transfer Certificate

5.

Original Hospital training Certificate

6.

Residential proof (Election ID/Aadhar card/Bank account/Indian Passport/ ration card/driving license)

7.

Fees payment (Rs.510/-by Cash)

8.

Recent 5 Passport size color photos

9.

Notarized Affidavit-A  

10.

Cloth lined 14" x 10" size cover affixed with Rs.40/- postal stamp. (Self-addressed as in Residential proof)

11.

Court Fee stamps for Rs.3 (to paste on the application)

III.

Fill the application and tag all the documents in sequence as per CHECKLIST

IV.

Submit application IN PERSON in the office of Andhra Pradesh Pharmacy Council.

V.

Get Originals endorsed & stamped to complete application process.
