Death Registration Process

Under the Registration of Birth & Death Act 1969, it is mandatory to register death of every citizen. The Municipal Register / CMO acts as the Death Register and is authorized to issue Death Certificate.

Types

Normal Death

Normal death means the permanent disappearance of all evidence of life at any time after live-birth has taken place;

Accidental Death

Death resulting from an accident (unusual event) that was unanticipated by everyone involved. A death may be considered "accidental" even if it was intentional or expected.

The applicant fills the form, attaches scanned copy of supporting documents while submitting the application form on-line and submits it on the portal.After online form submission, the system would calculate and show required fees to the applicant has to make online payment. Once the payment has been successful, the system will generate a registration number on the payment receipt and give a message confirmation to applicant to submit all documents in municipal office within next 5 working days.

The applicant takes the payment receipt and submits it in the ward office along with supporting documents in original

In case of death in the hospital, the hospital fills up the death form online with necessary information and submits it. The hospital hands over the form submission receipt to the applicant.

The applicant then takes the hospital receipt and submits it in the municipal office. The rest of the process is same as online application by an applicant as described above.

Supporting document to be submitted are cemetery receipt and an identity proof of the dead person provided by applicant to MP Municipal Office clerk

Identity Proof (any one of these) of dead person-

  1. PAN Card
  2. Aadhar Card/ SSSM Id
  3. Electricity Bill
  4. Property Tax Receipt
  5. Ration Card, (BPL, APL & AY)
  6. Election/Voter Card
  7. Driving License
  8. Passport

Death registration form provided by hospital.

Cause of death certificate from hospital.

FIR Copy (In case of death due to accident).

Identity proof (any one) of applicant submitted in the hospital at the time of death-
